I know here in Pennsylvania for example the Pennsylvania State Police had to be the ones to fingerprint you for your State of Pennsylvania Hazmat Endorsement on your drivers license. I would be willing to bet there is still a way to print them from the computer.   • This address will also be used for sending a hard copy of your document.
  • RESIDENCE OF PERSON FINGERPRINTED: Provide your address.
  • SIGNATURE OF PERSON FINGERPRINTED: Provide your signature or the signature of the person being fingerprinted.
